Dr. Michelle Kuisma is joining Mimico Medical! For Dr. Kuisma’s patients who are transferring from Westside Medical Clinic we are committed to making this transition as smooth as possible.

Below you will see a button that reads ‘Transfer My Care’. Click this button and you will be taken to the forms we require from you in order to transfer your care to Mimico Medical.

These forms can be filled out online either with your computer or smartphone. Be sure to fill in the fields that are marked ‘required’ and provide an e-signature at the bottom of each form.

Once we have received your forms, our team will add you to our clinic’s patient roster so that you can make appointments with Dr. Kuisma.
